Each child must have a completed form to participate.
The extra pose is available only for the customers that purchase the package before photo day. 
All sales are final, and packages are non-refundable.
Photos will be distributed at the daycare/school unless otherwise arranged.
By submitting the form, you give permission for your child to be photographed.
The parent/guardian is responsible for selecting images where applicable, within the provided timeframe.
